Community Based Day Services


Through community-based day services, D&S staff assists the service recipient to participate in meaningful and productive activities in various settings with other community members who may or may not have disabilities.  The services provided are geared toward enhancing personal growth and helping the service recipient feel connected to his/her community.  Therapeutic goals and objectives are required for service recipients receiving community-based day services.

 

Community-based day services may be provided in a service recipient’s home if there is a health, behavioral or other medical reason or if the service recipient has chosen retirement. 

 

Supports provided may facilitate job exploration activities, volunteer work, educational activities, and other meaningful, measurable community activities that promote developing relationships and maintaining ongoing relationships with friends and family members.

 

 

 

Community-based day activities may involve:
 
  • Exploring possible job sites
  • Doing volunteer work
  • Job shadowing
  • Visiting the library
  • Educational activities
  • Recreational activities
  • Art club
  • YMCA membership
  • Bowling
  • Volunteering at the Humane Society
  • Visiting Museums
Community-based day activities are individualized and aligned with the goals specified in the consumer’s individual support plan (ISP). D&S assures that day activities are geared toward enhancing personal growth.
