How Can I Become a Part of
the D&S Foster Care Family?
Opening your heart and home as a Foster Care Provider can have a
meaningful impact on the life of the disabled individual and that of
their family. Here is how we can help you get started:
1. Complete an application. You can pick up an application at one of
our local offices or download one using the link below

2. Meet with the D&S Foster Care Coordinator and review your home
and setting;
3. D&S staff will complete a background check;
4. Meet with D&S to discuss potential living arrangements;
5. Sign contract outlining our expectations and payment amounts.
